I am a Title IV ADA Specialist. I represent the Telecommunication Relay Service Fund and the Hard of Hearing. I go out and educate letting the Hard of Hearing know how they can get this Free Caption phone at no cost. Their are no activation or installation fees. I do all installs myself, and when completed I fill out a Medical Release form. They are sent to my Compliant group, and then they Contact the proper physician to have A Physician Certification form signed.
