PIEDMONT PHARMACEUTICAL CARE NETWORK, LLC (PPCN)

PPCN is an independent third-party contractor that has been providing chronic condition care management since the early 2000s.

HealthMapRx is a chronic condition care management program designed to reduce financial and clinical risks associated with prevalent chronic conditions. 

HealthMapRx programs are designed to help self-funded plan sponsors manage costs by reducing the risk of complications from common chronic conditions, including, diabetes, cardiovascular disease (hypertension, hyperlipidemia, and pre-diabetes), asthma, depression, anxiety and/or medical weight management (BMI>or=40).

HealthMapRx primarily uses clinical pharmacists and board-certified health coaches as care managers. These care managers meet with participants in private, by appointment for 3 initial monthly visits and then at least once per quarter thereafter depending on risk and condition control. Appointments are generally 30 minutes to one hour, most often at a location convenient to the participant at their worksite.
