Want help Managing your Type 2 Diabetes? In this study, you will …

  • Participate in 8 hours of classes and individual follow sessions and/or use a smartphone app for three months
  • Complete surveys and health measurements (e.g., A1c, blood pressure) at four time points throughout the study over 12 months
  • Eligible participants will receive up to $300 in gift-cards after completing the entire study
