Formerly Hospice & Palliative CareCenter, Trellis Supportive Care has provided care since 1979. We serve 13 counties from four offices. We are the owner and operator of the Kate B. Reynolds Hospice Home. We were the state’s first hospice provider and remain the largest. Some of our programs are: hospice care, palliative care, veterans’ services, advanced care planning, grief programs, community outreach, and more.
